Instructions:

  • Please answer the following prompts. You may select one of three methods to submit your responses.

Required Prompts:

  • What does MSHSL mean to you?
  • Why are you interested in becoming a member of the MSHSL Student Leadership and Advisory Council?

Choose Only One Prompt for the Following Below:

  • If selected to serve on the MSHSL Student Leadership and Advisory Council, explain the aspects of education-based activities you would you be motivated to improve. Why?
  • What would you like to accomplish at your high school before you graduate?
  • What questions do you have about the MSHSL that you would like to learn more about?

  1. Create and upload a video.  Videos may be up to 100 MB and in any of the following formats: avi mov mp4 ogg wav webm
  2. Attach a file of your written responses. Files may be up to 100 MB and in any of the following format: txt rtf pdf doc docx odt ppt pptx odp xls xlsx ods.
  3. Use the text box in the web form application to add text or a link to your posted video.
