This report may be submitted by an MSHSL Official to report praise or an incident.

Process for MSHSL Officials:

This is a publicly accessible document. League staff will review all submitted contest reports. Incomplete reports or reports requiring additional information will be referred back to the reporting official for revision.

1. Complete one report for each incident or praise.

2. Please ensure your report details the behaviors and conversations that took place and your response to those behaviors and conversations you observed and heard.
