This form is submitted by schools forming a cooperative.

Cooperative Sponsorship Dissolution Form

This form is submitted by schools dissolving a cooperative.

Activity Add/Drop Form

This form is used for schools adding or dropping activities. 

If the added activity is also in a co-op, the forms for co-ops need to be submitted also. 

Missing Activity Form

This form is used for schools that registered for an activity and it is not showing up on their school page. 

If the activity is in a co-op, it is likely due to one or more schools in the co-op that did not register for the activity.
