After the schools transfer determination is agreed upon by the MSHSL you cannot make any changes to this form.

Did the student previously attend your school in grades 9 through 12?
Did the student attend school or participate in a fall sport practice at the sending school for more than 15 calendar days before returning directly to your school?
Has the student used the "Return to Former School" transfer option in the past 365 days?
Does the student elect to decline the 15 day Return to Former School Policy that conditionally allows the student to return to the Sending School?
Have you contacted the student's parents to confirm the student's choice to decline the 15 day return option is accurate?
Is the family residence located in your public school district attendance area?
