Physical Forms
The 2023-2024 Sports Qualifying Physical Examination (SQPE) remains the most current form for use by our students and schools. This form is an acceptable form for use for students beginning their participation career and for those who are updating a previous physical examination. Each student is cleared for three years from the date of examination. The 2024-2025 SQPE remains under review at this time and will be posted upon completion. Please reach out to the League with questions.
